My grandmother living at home alone

My grandmother lives on her own at 91 but suffers with short time memory loss.

She wanders the streets late at night alone.

But I have been told because she refuses to go into a care home I cannot make her go in but she is getting too much for me.

I have carers going in everyday but they don't do anything for her so she is not washed or bathed or doesn't have clean clothes on everyday.

She leaves the door open when she goes out - she
Walks in the middle of the road.

I am at a loss who to turn to next.

I have talked to doctors, health workers and anyone else but i am not getting anywhere.

I do work for our local council on community care and i know how to take care of her but i have 3 children, a husband who works nights and i work as well.

HELP!
